What Happens After You Send the Referral
Step 1 — Screen for needs, resources & benefits
We screen the client for service needs and review available resources and benefit eligibility — VA Community Care, HBPC, Aid & Attendance, long-term care insurance, Area Agency on Aging, Community Memory Care programs, and private pay options — so the family knows what’s available before care is planned.
Step 2 — Intake call
An intake coordinator promptly contacts the family to confirm address, schedule, and any home environment concerns — pets, stairs, equipment, oxygen, etc.
Step 3 — In-home assessment
A supervisor meets the client in the home (or hospital room before discharge) to build the care plan and match the right caregiver. Most assessments happen quickly — often the same or next day.
Step 4 — Care begins
Care can start the same day as the assessment when needed. We send a confirmation back to the referring discharge planner with the caregiver name and start time.

How fast can you start care after a hospital discharge?
For weekday discharges with daytime notice, same-day starts are routine. For Friday afternoon or weekend discharges, we have begun care the same evening when an on-call supervisor is paged. The typical fastest path is: fax referral → intake call to family within the hour → in-home assessment → caregiver in the home.
Do you accept VA Community Care authorizations?
Yes. Embark Care is credentialed with both TriWest and Optum, the two third-party administrators of the VA Community Care Network. Use NPI 1437747730 on the authorization. We bill the network directly — the veteran and family have no out-of-pocket exposure.
What is the minimum visit length?
There is no minimum. Most home care agencies in this region require a 4-hour minimum shift. Embark Care does not — we will staff a 1-hour bath visit, a 2-hour mid-day check-in, or full live-in coverage, whichever fits the clinical need. This is the single biggest differentiator families and discharge planners notice.
Do you provide skilled nursing or wound care?
No. Embark Care is a Texas HHSC licensed Home and Community Support Services Agency (HCSSA #020526) authorized for Personal Assistance Services — non-medical home care only. We coordinate with the home health agency, hospice, or VA HBPC team handling skilled care. Many of our clients have both.
What private pay rate should I quote families?
$25 to $30 per hour depending on care level and location. No setup fees, no contracts, no minimum hours, no cancellation fees. Live-in and 24-hour rates available on request.
What about clients outside your service area?
If the family is outside our 43-Texas-county / 10-Arkansas-county footprint, we will connect them with a trusted home care agency in their area. We do not deliver care outside our licensed service area.
